Welcome to Trinity Veterinary Hospital in Lancaster, NY. Our New Client Form helps us gather essential information about you and your pet before your first visit, enabling our veterinary team to provide efficient, personalized care from the moment you arrive.

This form is required for all first-time clients visiting Trinity Veterinary Hospital. It collects basic contact details, patient information, and background needed to establish care for dogs, cats, and select exotic pets. Completing the form in advance helps reduce check-in time and ensures your pet’s medical record is accurate and complete.

Payment is due in full at the time that services are performed. If being admitted into the hospital, we cannot begin the care of your Pet until you have confirmed your desire to do so by 1) signing the client consent & estimate form, and 2) leaving an initial deposit of 50% of the upper end of the estimate. This is the only way that we have of knowing for certain that you want us to proceed with the care of your Pet. We accept Cash, Visa, MasterCard, Discover, and CareCredit payments. We neither extend credit, nor bill for services. All open invoices are sent to collections after 45 days unless prior arrangements are made. *

What to Expect After Submission

Once your New Client Form is submitted, a member of our team will review the information and add it to your pet’s medical record. If anything further is needed before your appointment, we’ll reach out directly. If you have not yet scheduled a visit, we recommend booking an appointment so we can properly match your form to your upcoming visit.
